Licenses & Permits
All license and permit applications may be mailed, emailed, or dropped off (unless otherwise noted).

Liquor License (New and Transfer) (PDF) | This is needed to apply for a new liquor license or a liquor license transfer. View the Plymouth Liquor Regulations (PDF) & Server Training Requirements Policy (PDF). Must be mailed or physically dropped off at the Town Manager's Office. |
Amplified Music for Business Application (PDF) | To be filed if you have outdoor music playing at your business. |
Amplified Music for Residents Application (PDF) | To be filed if you have outdoor amplified or live music. |
Auctioneer's Annual Permit Application (PDF) | To be filed if you will be operating an auction house. |
Auctioneer's Special Permit Application (PDF) | To be filed on an individual basis for each auction you will be holding. |
Auto Dealer License Application (PDF) | To be filed if you want a Class I, II, or III license. Required if you are buying, selling, or exchanging new or second-hand vehicles or assembling second-hand motor vehicles or parts thereof. |
Automatic Amusement License Application (PDF) | To be filed for use of any automatic amusement machine, device, or recreation. View the Automatic Amusement Policy (PDF). |
Liquor License Background Check Form (PDF) | To be filed if you are applying for a liquor license, a taxi operator license, a fortune telling license, or a pawnbroker license. This is part of the packet needed to apply for a liquor license. View the Plymouth Liquor Regulations (PDF). |
Blocking Parking Spaces Permit Application (PDF) | To be filed if you will be blocking parking spaces. |
Blocking Sidewalks Permit Application (PDF) | To be filed if you will be blocking a sidewalk or a portion thereof. |

Common Victualler License Application (PDF) | To be filed if you are an establishment that will be serving and preparing food. |
Comprehensive Entertainment License Application (PDF) | To be filed by a business that will have any type of entertainment including but not limited to: Radio, TV, Jukebox, Music, Dancing, Plays, Moving Pictures, Light or Theatre Shows. Early Sunday Opening Application (PDF) | To be filed if you are applying for a liquor license and wish to start serving alcohol at 11:00 a.m. on Sundays as opposed to 12:00 p.m. View the Plymouth Liquor Regulations PDF). |
Extension of Hours Application (PDF) | To be filed if you are applying for a liquor license and wish to serve alcohol until 1 am as opposed to midnight. View the Plymouth Liquor Regulations (PDF). |
Fortune Telling Application (PDF) | To be filed for any person and/or business that will be doing any type of fortune telling including but not limited to fortune telling, palm reading, card reading, etc. |
Horse and Carriage Permit Application (PDF) | To be filed if you will be operating a horse-drawn carriage on public roads and ways. View the Horse Carriage Services Rules & Regulations (PDF). |
Innholder License Application (PDF) | To be filed for anyone offering to the public lodgings and food. |
Junk Dealer's License Application (PDF) | To be filed for anyone for the purchase, sale, or barter of junk, old metals, or second-hand articles. |

Lodging House License Application (PDF) | To be filed for anyone that will be operating a house where lodgings are let to four or more persons, not within the second degree of kindred to the person conducting it. |
Moving Buildings Permit Application (PDF) | To be filed if you are moving buildings in the Town of Plymouth. |
One-Day Liquor License Application (PDF) | To be filed for the sale of alcoholic beverages to the public. One-day license for the sale of all alcoholic beverages may be issued to non-profit organizations only. All others may only get a wine and malt one-day license. Pawnbroker License Application (PDF) | To be filed for anyone whose business or occupation includes the taking or receiving, by way of pledge or pawn, of any article or personal property as security for the payment or repayment of money. |
Pool Table License Application (PDF) | To be filed for any establishment to keep a billiard, pool, or sippio table. |
Revenue Enforcement and Protection (REAP) Tax Form (PDF) | To be filed for any business licensed in the Town of Plymouth. |
Sunday Entertainment Application (PDF) | To be filed if you are a business that will have entertainment including but not limited to: Radio, TV, Jukebox, Music, Plays, Moving Pictures, and Light or Theatre Shows. |
Under-Above Ground Storage (PDF) | To be filed for any land that has above or underground storage of flammable liquids. |
Worker's Compensation Form (PDF) | To be filed for any business licensed in the Town of Plymouth. |
