Building bridges to community wellbeing, the mission of the Plymouth Department of Health and Human Services (HHS) is to connect people with vital resources within the framework of a healthy, humanistic, sustainable, and resilient community. Our goal is to foster a comprehensive and compassionate social health support system that promotes a culture of belonging and cohesiveness for residents and visitors alike.

Plymouth HHS envisions a systematic approach to ensuring health equity for all with a commitment to excellence, community engagement, supportive partnerships, collaboration, and innovation in America's hometown. Plymouth HHS is comprised of five divisions listed below, including the Center for Active Living (CAL), Plymouth Public Library, Public Health Division, Recreation Division, and Veterans Services Division (see navigation tabs on left).

The Overnights of Hospitality is a partnership between the Plymouth Task Force to End Homelessness and Father Bill's & Mainspring. Your Voice Matters! Take the Plymouth Community Health Survey...

The Town of Plymouth is launching the first-ever Plymouth Community Health Survey as part of the inaugural Plymouth Community Health Assessment & Community Health Improvement Plan (CHA-CHIP). Through the Plymouth CHA-CHIP Project led by HHS, this survey is a unique opportunity for people who live and/or work in Plymouth to share their feedback on health and wellbeing in our community.

Age and Dementia-Friendly Plymouth (ADFP) Mini-Hub


The Town of Plymouth was recently awarded official designation as an Age–and Dementia Friendly Community by Dementia Friendly America Massachusetts and AARP. This designation provides resources for communities to inform, promote, and prioritize improvements that benefit residents across all ages and life stages. From 2021-2024, the Town of Plymouth’s Senior Task Force worked closely with the Gerontology Institute at UMass–Boston to develop Age–and Dementia Friendly Plymouth (ADFP), a community needs assessment, engagement, and action planning initiative that explores the needs and strengths among Plymouth residents for healthy aging.

As the bridge to resources and in support of ADFP, Plymouth HHS has created a mini-hub below for information on healthy aging, dementia awareness, and family support at the local, state, and national levels. We encourage you to learn more about ADFP, Age–and Dementia Friendly Communities, and these resources so that you can be informed, engaged, and support your loved ones/neighbors in healthy aging:
